At Dogwood Dental, we want you to be able to make the most out of your time with Dr. Eric Jacobs and our team. When you call our office for your appointment, we will email your new patient forms to you. We ask that you complete them and return them to our office as soon as you can.
Not only will this save you time on the day of your visit, but it will also allow our dentist to study your medical history and begin creating potential treatment plans.
